  • Tim Perez

    PMHNP-BC

    Tim Perez is a Board Certified Psychiatric Mental Health Nurse Practitioner. Tim sees patients of all ages; however, he specializes in the diagnosis and treatment of children and adolescents for psychiatric disorders including ADHD, Inattention, Depression, Anxiety, Bipolar disorder, and Behavioral disorders. Prior to joining Valueris, Tim spent 6 years working with children and adolescents at Marion and Polk counties. He has worked extensively with at-risk children, youth and adults in the treatment and management of severe and persistent mental illnesses, acute crises, and the recognition and prevention of suicide. Tim and his wife live in the Portland area and have been married for over thirty years. They are the parents of five children and eight grandchildren.

  • Nate Kravitz

    RN, MN, PMHNP-BC

    Nate Kravitz is a Board Certified Psychiatric Mental Health Nurse Practitioner. Nate is at our Salem location. He primarily sees adults for depression, anxiety, addictions, psychosis, bipolar disorder, and schizophrenia. He hopes to help others meet their health goals so they can meet their life goals with family, work, school, relationships, recreation, and faith. Prior to joining Valueris, Nate worked for 16 years in residential addictions treatment, and 7 years with community mental health. Nate has been married for 11 years and has two children. His hobbies include running, playing with his children, and playing Ultimate Frisbee.

  • Lucy Bloedon

    MN, PMHNP-BC

    Lucy is a Board Certified Psychiatric Mental Health Nurse Practitioner. She sees ages between 5 and 64 and specializes in diagnosis and mediation management for psychiatric disorders including depression, bipolar disorder, anxiety, ADHD, sleep disorders, and schizophrenia. She has worked in long-term care as well as in acute inpatient care settings. Lucy earned her MSN in Nursing from Gonzaga University, her BSN in Nursing from OHSU, and has a BA in Psychology from Colorado College. Prior to becoming a PMHNP, Lucy worked for almost seven years as a Registered Nurse and has worked in mental health settings since 2006. Lucy’s ideal client is already in therapy and is looking to add medication management. Lucy will do some supportive therapy influenced by CBT, DBT, Motivational interviewing, Person Centered Therapy, and Existential Therapy. Her approach is trauma informed and inclusive for all. She partners with you to support you in living your best life.

  • Sohyon Rahe

    MS, PMHNP-BC

    Sohyon Rahe is an Asian American Board Certified Psychiatric Mental Health Nurse Practitioner who has over 30 years of experience. She specializes in treating primarily adults and provides services to whole range of psychiatric illnesses and emotional challenges. Her specialty includes but not limited to Depression, Anxiety, Bipolar I and II, ADHD, OCD, Neurocognitive Disorder including Alzheimer's and other Dementia, and Psychoses. She also has a degree in Psychology which enables her with an additional avenue to promote healing from trauma, self-esteem, and improve grief and loss, and more. She believes in collaborative treatment approaches between a provider and a client by inviting active participation from clients. Off time, she enjoys reading, travelling, swimming, Opera, Broadway, Museums and Japanese Gardens.

  • Ursula White

    MS, PMHNP-BC

    Ursula is a Board Certified Psychiatric Mental Health Nurse Practitioner. She sees adults ages 18 and over and provides services for those with a whole range of psychiatric illnesses and emotional challenges. She has an emphasis on Bipolar I and II disorders and treatment resistant Depression. She has worked in community mental health for the past ten years while also running a private practice. She works with people who may only need short term treatment with medication and also with people who may need longer treatment for chronic conditions. She graduated from OHSU in 1991 with a BSN and worked for 20 years as hospital nurse before returning to graduate school at OHSU to earn a Master degree and to become a Psychiatric Nurse Practitioner. She hopes that with medication management, you will feel more like yourself and it will help you to rediscover, restore, and retain your sense of individual value or worth. In her spare time, she enjoys travel and spending time with family and friends.

  • Roger Tunks

    MN, PMHNP-BC

    Roger Tunks is a Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP). Roger sees patients from school-aged to 64, specializing in people struggling with focus, inattention, depressive thoughts, anxiety, and mood irregularities. He has 8 years of nursing experience working in emergency departments and ICUs. It was in those environments that he witnessed first-hand the effects of people struggling with mental illness who were unable to get the support before it became a crisis. Realizing the need for mental health services, he continued his education as a PMHNP. His hope is to help people understand what is causing their struggles and how to achieve a higher level of mental health before it becomes a crisis. Roger earned his MSN in nursing with a focus in psychiatric mental health from Gonzaga University. Roger views psychiatry as a collaboration where the patient brings the knowledge of their lived experiences and unique symptoms while he contributes his knowledge of mental health, diagnoses and treatments. Together they are able to create an individualized treatment plan. When Roger is not helping care for patients, he enjoys long distance running, skiing, cooking, downhill mountain biking and spending time with his wife and two children. Roger is currently accepting new patients and works hard to see them within a week of a request.
