Please give your insurance provider a call to get an explanation of your mental health benefits as those often differs from the medical coverage on your plan. This may include a different co-pay, co-insurance, or deductible.
If you do not get that information, we will not be aware of what your co-payment or co-insurance will be until after we have billed insurance.
For current patients, we ask that you arrive 10 minutes early for your appointment to check in at the front desk. Co-pays and co-insurance payments are due at check-in. A $50 fee will be charged for payments not made the same day as the appointment.
If you are no longer able to make this appointment, please contact us at 503-389-1500 with at least 2 business days notice, and we will be more than happy to reschedule it with you. Shorter-term cancellations may result in a $100 fee at your provider’s discretion.
For new patients, we require seven business days notice if you need to cancel your first appointment or the deposit is forfeited.
If you deem it unsafe to come to your appointment due to inclement weather, please call us at 503-389-1500 as soon as possible to let us know. If you get our answering machine, please leave a detailed message. We can reschedule your appointment or arrange for a telemedicine (video chat) visit during your originally appointed time. We can walk you through how to set up for this video chat.
