For current patients, we ask that you arrive 10 minutes early for your appointment to check in at the front desk. Co-pays and co-insurance payments are due at check-in. A $50 fee will be charged for payments not made the same day as the appointment.
If you are no longer able to make this appointment, please contact us at 503-389-1500 with at least 3 business days notice, and we will be more than happy to reschedule it with you. Shorter-term cancellations may result in a $100 fee at your provider’s discretion.
For new patients, please refer to the cancellation policy included in the reminder email sent to you for your first appointment.
If you deem it unsafe to come to your appointment due to bad weather, please call us at 503-389-1500 as soon as possible to let us know. If you get our answering machine, please leave a detailed message. We can reschedule your appointment or arrange for a telemedicine (video chat) visit during your originally appointed time. We can walk you through how to set up for this video chat.
