COVID-19 update: We are having all new patient appointments be done through a Telemedicine appointment. Please read the section on Telemedicine for information regarding what to do. In addition, we ask that you take a picture of your ID and Insurance Card (front and back) and send it to our email [email protected] or text it to our office number 503-389-1500. We hope you are staying safe during this time.
For new patients, extensive paperwork will be uploaded to the portal to be completed before your initial appointment. If you cannot access the patient portal, we can also send you the new patient paperwork through the mail.
Scheduling
If you want to schedule a new patient appointment with one of our providers, please contact our office at 503-389-1500. One of our office staff will pick up the phone and will help get you scheduled.
Insurance
Please check the insurance section on our website for a list of currently accepted insurances for each provider.
Cancellation and No-Show Fee
At the time of scheduling your first appointment we will take payment information to keep on file. We ask for one-week’s notice for cancelling or rescheduling your first appointment. Should you miss your first appointment without notice to our office, a $200 no-show fee will be charged to the card on file.
At The First Appointment
At your first appointment, we expect for you to have filled out the new patient paperwork. If you cannot or if you were unable to, please let us know before hand and we can have you come early to fill it out at our office.
We will also have you sign any Releases of Information that you have filled out, and also get a copy of your ID and Insurance Card (front and back) for your patient chart. You will not be prevented from seeing your provider because you forgot to bring your ID or Insurance Card with you. We will ask that you bring it to a future appointment so we can get a copy of it.
Standard release of information form is required before we can send your confidential patient information to those you specify.
At least ten minutes prior to your telemedicine appointment, fill out your RPQ (returning patient questionnaire) in the Patient Portal then, please call us at 503-389-1500 to make all required payments. Then, using either the Chrome, Safari, or Firefox web browsers, on your computer, smartphone, or tablet, go to the link below to access the waiting room of your provider:
